A recent article in the Seattle Times profiled the results of Qualis Health’s Stepping Stones: Bridging Health Care Gaps (The Care Transitions Project of Whatcom County), a pilot project aimed at reducing avoidable hospital readmissions in Whatcom County.

From 2008-2011, Dumas & Associates, Inc. provided independent facilitation to the Qualis Health – Whatcom County Stepping Stones Project leadership team, which directed the study pilot design, identified priority gaps, and designed and evaluated the strategic interventions. Project participants developed a patient-centric framework to support assessment of current available data and convened healthcare providers, patients, and broader community stakeholders in providing input on the relationships and networks necessary to reduce hospital readmission rates. The Whatcom County pilot achieved the lowest readmission rate of the 14 pilots conducted across the U.S., a great start to the long road ahead to address changing health care policy in our country and our communities.
